Our physicians and providers participate in most insurance plans and we will submit claims to these companies on your behalf.  Since we do not have knowledge of the details of every insurance plan available, we must rely on you to determine if we are participating providers with your insurance.  Since every plan is different, please be sure to check your coverage if you have questions as to what your insurance will cover or pay for.

To enable us to process your bill accurately, we will ask you for your insurance card and check your picture identification each time you visit. 

We are obligated by your insurance company to collect co-payments at each visit.  We accept cash, checks, debit cards and all major credit cards.  Please be aware that your co-payment could increase without a new card being issued to you.  Your insurance company can tell you your current co-payment amount.  If you do not pay your co-pay at the time of your visit, a $20.00 service charge will be applied to your account to cover the costs of billing and processing. 

We will process your medical claims directly to your insurance carrier for you.  While we make every effort to resolve insurance issues, please remember that you are ultimately responsible for your insurance benefits, as well as the cost of your health care.

Once your insurance company has determined the portion of our bill that you are required to pay, we will mail a statement of your account to you.  Many people have insurance policies with only partial coverage. This may mean that you will pay more out of your own pocket than you have in the past. 

All accounts are due upon receipt of our statement.  A finance charge of 0.666% per month (8% annual percentage rate) will be added to any accounts over 30 days old. 

Please contact our business office if you need to make special arrangements for payment.  Our staff is very knowledgeable and experienced and will make every effort to assist you.  Unfortunately, we cannot continue to extend credit to patients who are unwilling to keep their accounts current.

Appointment Cancellation Policy

If you need to cancel your appointment, please notify our office 24 hours before your appointment (please contact us on Fridays for Monday appointments).  Receiving cancellation information in advance allows us to schedule and serve other patients.  We reserve the right to bill you $50 for a missed appointment without a 24-hour advanced notice.  This fee is not covered by insurance carriers or Medicare and will be your responsibility to pay before your next visit.

Forms Completion

Our physicians may charge for completion of forms, for which they may request a pre-payment at the time the form is received.  Our fees are available upon request.

What is a deductible?

A deductible is an amount that you are required to pay out of your own pocket up to a certain limit before a higher level of insurance benefit is available to you.  Although not everyone has a deductible policy, they are becoming more common.  A deductible can be as small as $50 or as large as $5,000 or more.

I have insurance coverage so why did I get a bill?

Please review any bill you receive from us carefully.  If you have questions, please call your insurance company or our Customer Service Team at 763-302-8282, ext. 2017, Monday through Friday, 8:00 a.m.-5:00 p.m., for assistance.

Medical Assistance

We accept Medical Assistance.  If you have applied for Medical Assistance, please contact our Patient Account Representative at (763) 302-8282, ext. 2017, with the date of application and your social worker's name and telephone number.  Once you have been approved for Medical Assistance, please contact our Patient Account Representative with your ID number and effective date.

Automobile and Other Liability Claims

Please bring the following information to each appointment:

• Date of accident
• Name and phone number of your claims adjuster
• Claim number
• Name, address, and phone number of the insurance company
• Information regarding other health insurance coverage you carry, should your claim be denied.

Workers' Compensation

Please bring the following information to each appointment:

• Date of accident
• Name of your employer
• Workers' Compensation claim number
• Name and hone number of your claims adjustor
